To ensure a seamless and secure transaction process, all required documents must be submitted together as a complete package. Partial or separate submissions will delay your application. Please adhere to the guidelines below carefully.

Submit all documents at once — individual or email submissions are not accepted.

All documents must be clear, legible, and properly signed.

Signatures must be in blue wet ink; electronic or copied signatures are not permitted.

Signature authentication by seal, fingerprint, or notary public is mandatory where applicable.

| 🏦 | Bank Statement | Submit a recent, clear color copy confirming entity status and sufficiency of funds for closing costs. |
| 🏠 | Utility Bill | Provide a recent color copy as proof of the client’s address. |
| ✍️ | Signed Smart Plan Agreement | A signed copy of this agreement authorizes USCPB to engage with you. Signing does not imply payment obligations unless services are requested. |
| ✅ | Authorization to Verify | Signed authorization permitting USCPB to verify submitted documents in compliance with KYC regulations. |
| 👤 | Confirmation of Principal and MFPA | Confirmation from your New Client Engagement package affirming principal party status and intermediary fee agreements. |
| ✒️ | Signature Card | Notarized copy of your banking signature card for verification purposes in all transactions. |
| 💰 | Asset Monetization Questionnaire (If applicable) | Required if assets are to be used as collateral backing bankable instruments. |
| 📊 | Proof of Asset Value (If applicable) | Appraisal reports, assays, videos, or other documentation validating asset value for monetization or sale. |
| 🔗 | Proof of Chain of Custody (If applicable) | Legal ownership and transfer documents, including HD videos and photos with a dated newspaper and USCPB letterhead for authentication. |
